DECLARATION

Note: for the purposes of this declaration the Lebanese Moslem Association will signify all sub‐branches, employers, groups and members.

1) I, the undersigned as a condition of acceptance of my entry to the above event, for myself, my heirs, executors and administrators, herby waive all and any claims, fight of cause of action, which I or they might otherwise bring out of any loss of life or injury, damage or loss of any description which I may suffer or sustain in the course of or consequent upon my entry to the said event;

2) This waiver, release and discharge shall be and operates separately in favour of all persons, corporations, and bodies involved or otherwise engaged in promoting or staging the event;

3) All persons participating in a class or a camp conducted by The Lebanese Moslem Association must agree to be bound by the Code of Conduct as set by the Lebanese Moslem Association . The Lebanese Moslem Association reserves the right to make amendments or additions to this Code of Conduct at any time. If at any time the person participating in the camp does not follow any rules, we will call his parents to come pick him at his cost.

Risk Warning
1) I understand Sports and Recreational activities can be risky and may cause any number of injuries to participants.

2)By Signing this authority, I agree that no Liability is conferred onto the organisers, and I also agree to indemnify the Lebanese Moslem Association and organisers involved from any legal action or litigation as a result of any injury, death or loss that may result from this activity.
